Orders shipped to Canadian addresses need to have certain
costs determined prior to completing the order. Please
use the form below to place a "pre-order"
with us. We will send you an email detailing the costs
and instructions on how to place your order on our site.
Filling out this form does not commit you to paying
for these items or the associated shipping costs to
Canada. We are only gathering your information to provide
you with a quote for a final invoice amount.

of this information:
Once you have filled out this form, your
pre-order will be emailed to us and a copy will be emailed
to you. Save
the copy of this pre-order email for future verification.
After clicking the "submit" button below,
you should see a "success" web page within
a few seconds. If you do not see this "success"
page and/or see a "failure" page, please fill
out this form again and resubmit it. If
you see a "success" page, you should receive
two emails from us within 15 minutes (a copy
of the pre-order and an autoresponder message). Any
email sent to us is received by an autoresponder, which
means that it automatically replies with an email simply
stating that we have received your email.
If you do not receive
these two emails from us, there are two possible problems.
First, there is some type of delay that may clear up
within a few hours. The second possibility is that you
provided us with an incorrect email. You can use our
basic "Contact
Us" page to ask if we received your pre-order.
We will look at your pre-order to make sure that we
understand what you want and that you have provided
all of the other required information. We will send
you an email that will contain the calculated shipping
and handling costs, as well as instructions on how to
place your order on our site. You should receive this
email within 3 business days. For
your protection and ours, we only ship to your PayPal
verified addresses.
This form does not do some of the automatic processing
done by our shopping cart. Therefore, we recommend that
you double-check the item name(s) and price(s), and,
your math for the grand total. If you wish, you can
select your items and fill your shopping cart, then,
print the web page of your final shopping cart to help
with filling out this form (if printing a web page is
beyond your computer skills, don't worry about it).
Make sure to fill
in at least the item name, unit price, and, quantity
for every item you are ordering. Remember that all money
is stated in U.S. Dollars.
